Assessment is a generic term to cover all kinds of testing types, including: skill assessments, tests, quizzes and anything you can think of to pose a question to a partcipant and receive, score and analyze their responses.

The primary difference between an assessment and a survey, is that an assessment is graded.  There is always a correct answer.  Surveys simply collect peoples thoughts, responses, opinions and such...there is no correct answer.
